SIGNING UP FOR HOSPICE

A referral to hospice can come from a patient, a caregiver, relatives and friends, a physician, a hospital discharge planner, or a nursing facility social worker.

A hospice consultation to discuss the philosophy, criteria, and services of Holy Cross Hospice is done to inform the patient and caregivers and see if hospice is right for them.  The patient’s physician gives confirmation of the patient’s life-limiting illness.  Once enrolled under hospice, a nurse will come to do the admission visit and initiate the plan of care with the patient, family, and team.
